This service allows you to notify us of a seatbelt exemption, due to a medical condition or disability.

You can use this service to notify us of a seatbelt exemption for yourself or someone else.

About seatbelt exemption certificates

Seatbelt exemptions can be provided by your doctor in extremely rare circumstances. If you have a genuine disability or medical condition that prevents you from wearing a seatbelt, consult with your doctor to be assessed for a seatbelt exemption certificate.

Seatbelt exemption certificates are not valid more than 12 months after date of issue. Once your certificate expires, you must visit your doctor to be re-assessed.

Before you use this service

There are many important requirements that must be met before submitting your seatbelt exemption certificate. To ensure you and your doctor have correctly met these requirements, read more about seatbelt rules and exemptions.

To use this service, you will need to provide:

  1. Customer details (including customer reference number, for existing customers)
  2. Contact information (an email address or phone number)
  3. A completed, signed Seatbelt Exemption Certificate (Form F2690). 
    Your certificate must:
  • be signed by your doctor 
  • display the date of issue 
  • display an expiry date of no more than 12 months after issue
  • state that, in the opinion of your doctor, you should not wear a seatbelt due to a medical condition or disability.
