About this service

This service allows you to notify us of a seatbelt exemption, due to a medical condition or disability.

You can use this service to notify us of a seatbelt exemption for yourself or someone else.

Seatbelt exemptions apply for a maximum of 12 months. Find out more about seatbelt rules and exemptions.

You will need to provide:

  1. Customer details (including customer reference number, for existing customers)
  2. Contact information (an email address or phone number)
  3. A completed, signed document from a doctor

Documents that we will accept:

  • Exemption from Wearing a Seatbelt Certificate (Form F2690)
  • Medical Certificate for Motor Vehicle Driver (Form F3712)
  • A letter from a doctor that includes a date of issue and an expiry date
