Notify us of a medical condition

About this service

You can use this service to notify the Department of Transport and Main Roads about a permanent or long-term mental or physical condition that is likely to affect your ability to drive safely.

You don't need to tell us about a temporary medical condition.

Are you 75 or over?

You only need to notify us if you have a medical condition that affects your ability to drive safely. Otherwise, you don't need to record your medical certificate with us.

Before you start

Please make sure you have ready:

  • your customer reference number—this is shown on the front of your driver licence
  • your medical certificate (not required if you are only updating your need to wear glasses or contact lenses to drive).

If you have trouble using this service, you can also notify us by email, by post or in person.

Are you a health professional?
Please use our service for doctors and health professionals to report a patient's medical condition.

Uploading a medical certificate

You will need to upload a medical certificate if:

  • you have not reported the medical condition to us before
  • a condition that you have reported to us before has gotten worse.

The medical certificate must be:

  • completed and signed by your doctor
  • a PDF, image file or Microsoft Word document
  • under 2MB in size.

Important information

You may choose to notify us of a medical condition before you've seen your doctor. This will meet your legal responsibility

However, if you do not upload a medical certificate when using this service your licence will be suspended until you give us a current medical certificate.

Note: If you are only using this service to report that you need to wear glasses or contact lenses to drive, you don't need to upload a medical certificate.
