Before you start

You can either complete this service for yourself or a parent, carer or agent can act as a representative for the applicant (the person with the functional impairment).

The applicant must have a customer reference number (CRN)
To use this service, the applicant must have a CRN issued by Transport and Main Roads (for example, a QLD driver licence, photo ID card or registration in their name).

If the applicant doesn't have a CRN, you'll need to visit us in person to apply for a permit (you'll need to bring evidence of identity documents for the applicant).

If you don't know the applicant's CRN, you can message us or call 13 23 80. We may be able to provide the CRN if we're able to successfully identify the applicant.

What you'll need

  • The applicant's customer reference number (CRN)
  • The applicant's name and date of birth
  • The address permits should be posted to

Note: If you have an existing disability parking permit, you can provide the permit number instead of the CRN.

  Replacing a permit

You'll need to know the full permit number to request a replacement if it's been lost, stolen or damaged.

Not sure of the permit number?
Please call 13 23 80 during business hours to arrange the replacement.
